Key takeaways
- GLP-1 drugs entered the public eye as weight-loss shots, but the science underneath them is much bigger.
- In the SELECT trial, semaglutide cut major heart events by 20%, largely independent of how much weight was lost.
- GLP-1 therapy also calms inflammation, helps blood vessels relax, and protects the kidneys.
- Fast dosing, muscle loss, and rebound weight gain are real risks without medical oversight.
- Arbour Longevity's supervised program is $299/month with GLP-1 therapy included, after a $35 first visit.

What GLP-1 actually is
GLP-1 receptor agonists (semaglutide, tirzepatide, and the wider class) became famous as weight loss drugs. The science underneath them is much bigger than that.
GLP-1 (glucagon-like peptide-1) is a natural hormone your gut makes after you eat. It tells the pancreas to release insulin (the hormone that moves sugar from blood into cells). It slows how fast the stomach empties, turns down glucagon, and switches on the fullness centers in your brain. Man-made GLP-1 drugs copy this hormone but last far longer in the body. That allows a once-a-week dose.
What the SELECT trial actually showed
SELECT (Semaglutide Effects on Cardiovascular Outcomes in People with Overweight or Obesity) was published in NEJM in November 2023. It randomized 17,604 adults aged 45+ with known cardiovascular (heart and blood vessel) disease and a BMI of 27 or more, but no diabetes. Mean follow-up was 39.8 months on semaglutide 2.4 mg weekly. The main outcome combined heart death, non-fatal heart attack, and non-fatal stroke. It occurred in 6.5% of the semaglutide group versus 8.0% of the placebo group, a 20% relative reduction (HR 0.80, P<0.001).

Notably, the heart benefit did not depend much on how much weight was lost. Patients who lost less weight still saw heart protection. The benefit held no matter the starting HbA1c (average blood sugar over three months). Semaglutide is not just a weight loss drug. It is a heart treatment.

The mechanisms beyond weight loss
- Direct anti-inflammatory effects. GLP-1 receptors sit on blood-vessel lining cells, vessel muscle cells, and immune cells. Many studies show lower inflammation markers (TNF-alpha, IL-6, hs-CRP) on GLP-1 therapy.
- Better blood-vessel function. Brachial artery flow-mediated dilation, a test of how well arteries relax, improves on GLP-1 therapy.
- Plaque stabilization. Some imaging studies show plaque in the heart arteries holding steady or shrinking a little.
- Direct effects on heart muscle. GLP-1 receptors on heart cells may help the heart relax between beats and cut heart failure events.
- Kidney protection. Less protein leaks into the urine (albuminuria), and kidney function declines more slowly in patients with diabetes and chronic kidney disease.
The longevity-medicine framing
In longevity medicine, a small set of shared drivers speeds up aging. These are chronic low-grade inflammation, insulin resistance (cells that stop responding to insulin) with high fasting insulin, deep belly fat, and stiff blood vessels. GLP-1 therapy acts on all four. That is why longevity clinics now build GLP-1 therapy into wider metabolic programs.
Why GLP-1 should not be taken without supervision
- Dose titration (raising the dose step by step) matters. Going up too fast causes bad stomach and gut symptoms.
- Drug clashes and contraindications (reasons not to use a drug) need screening.
- Some patients lose muscle along with fat if protein and strength training are skipped.
- Rebound weight gain after stopping is common without a structured maintenance plan.
- Lab checks (lipids, HbA1c, kidney function, vitamin and nutrition markers) support long-term success.
At Arbour Longevity, our medical weight loss program is $299/month and includes GLP-1 therapy and ongoing clinician oversight.
Frequently Asked Questions
Will I regain weight when I stop?
Without a structured maintenance plan, yes. Weight regain after stopping GLP-1 is common. With coaching support (food, strength training, and sometimes a lower upkeep dose), lasting results are within reach. We have an in-house ACE-certified health coach.
Will I lose muscle along with fat?
Some patients do. The risk drops when you add strength training and enough protein from the start of therapy.
Are compounded GLP-1 medications safe?
Compounded (pharmacy-mixed) versions vary widely in quality and oversight. At Arbour Longevity, we use FDA-approved products through pharmacies with verified sourcing.
