Surgical blepharoplasty removes excess eyelid skin by cutting it away and closing the wound with sutures. It works well, and for significant excess skin it remains the right answer. But it involves an operating room, anesthesia, incisions, and a recovery measured in weeks.
Non-surgical blepharoplasty — sometimes called plexrplasty — achieves skin tightening through a different mechanism entirely. A 0.5mm plasma probe creates a precise grid of microscopic sublimation points across the excess eyelid skin. Each point removes a microscopic column of tissue without cutting, and the surrounding skin contracts immediately toward those points. Over the following weeks, fibroblast activation lays down new collagen and the tightening continues.
There is no incision, so there is no suture line and no surgical scar. The eyelid skin is tightened in place rather than excised.

These are different procedures suited to different patients, not competing versions of the same thing. If you have significant excess skin, fat pad herniation, or eyelid skin obstructing your visual field, surgical blepharoplasty is very likely the better option and we will tell you so at consultation rather than sell you a treatment that will underdeliver.
What the Evidence Shows
Plexr Plus is an FDA-cleared clinical device supported by more than 40 peer-reviewed publications. In blinded clinical assessment, published evidence reports that roughly 90% of patients treated for mild to moderate excess eyelid skin demonstrated meaningful improvement.
That is a strong result, and it comes with an important qualifier: it applies to mild to moderate excess skin. The evidence does not support plasma as an equivalent to surgery for severe dermatochalasis or for fat pad removal. Response also varies with skin quality, age, and individual healing.
Non-surgical blepharoplasty is a cosmetic procedure. It is not a treatment for ptosis, thyroid eye disease, or any medical condition of the eyelid, and it is not a substitute for ophthalmologic evaluation where that is indicated.
The Procedure, Start to Finish
1 · Candidacy consultation
We assess skin laxity, tissue quality, fat pad position, and whether your concern is skin, fat, muscle, or brow position. Not everyone who asks for this procedure is a candidate for it, and that assessment happens before anything is scheduled.
2 · Numbing
Topical anesthetic is applied and given 30 to 45 minutes to take full effect. ProNox is available if you want additional comfort.
3 · Treatment
A precise grid of plasma points is applied across the treatment area. Upper lids typically take 20 to 30 minutes. You will see tightening immediately.
4 · Healing
Small carbon crusts form at each point and shed naturally over 3 to 5 days. Swelling is most noticeable on days one and two, particularly on waking. Makeup can generally resume once crusts have fully shed.
5 · Result development
Initial tightening is visible immediately. Collagen remodeling continues over 6 to 12 weeks. Depending on starting laxity, a second or third session at 6 to 8 week intervals may be recommended.
Do not pick the crusts. Allowing them to shed on their own is the single biggest determinant of a clean cosmetic result, and picking is the most common cause of pigmentary change or scarring.
Safe for Deeper Skin Tones
Laser and IPL devices target melanin as a chromophore, which is why periorbital laser resurfacing carries elevated risk of post-inflammatory hyperpigmentation in Fitzpatrick types IV through VI — and why many patients with deeper skin tones are turned away.
Plasma energy does not rely on chromophore absorption and does not preferentially target melanin. Under proper protocol it is appropriate across the full Fitzpatrick range. If you have been told you are not a candidate for laser eyelid treatment because of your skin tone, this is worth a conversation.
Post-inflammatory hyperpigmentation is still possible in any skin type after any controlled-injury procedure. Strict sun avoidance during healing materially reduces that risk.
Who Should Not Have This Procedure
- Significant excess eyelid skin or fat pad herniation — surgical blepharoplasty is more appropriate
- Eyelid skin obstructing the visual field — this warrants ophthalmologic evaluation, and may be a covered functional procedure
- Ptosis, where the issue is muscle position rather than skin
- Active autoimmune conditions affecting skin or healing
- Pregnancy or breastfeeding
- Diabetes — impaired healing requires individual assessment
- Anticoagulant or blood-thinning medication
- Active infection or open lesions near the treatment site
- History of keloid or hypertrophic scarring
- Recent isotretinoin use
- Herpes simplex in the treatment area without antiviral prophylaxis
Honest candidacy assessment is the whole job. A patient treated with plasma who needed surgery gets a disappointing result, and that helps nobody.
Traveling From Elsewhere in Michigan
